SBIRT Case Manager
Family Medicine - COM
Job Summary/Basic Function:
Available Sept. 1, 2013. Position is contingent upon grant funding and is located at the Kingsport Family Medicine Residency Program Clinic.
The Case Manager will provide services as part of the SBIRT Tennessee Program, a grant funded project to implement and evaluate an alcohol and drug cessation program. This person will provide and coordinate care for patients in outpatient practices, especially with respect to those at risk for alcohol and drug abuse.
-Works individually with patients.
-Coordinates care through contact with providers and agencies.
-Collects required project data.
-Tracks patients, providers, and manages related databases.
Knowledge, Skills, and Abilities:
-Knowledge of community resources to address alcohol and drug abuse.
-Knowledge of word processing and database management software.
-Superior organizational skills.
-Excellent interviewing and communication skills.
-Bachelor's degree in psychology, sociology, social work, public health, or related field.
-Two years direct work with individuals receiving medical care or social services.
-Must hold state license or certification in either general or substance abuse-specific counseling or therapy.
Master's Degree and two years of health behavior counseling experience is strongly preferred.
Family Medicine Clinic employees may be required to report at their regularly scheduled times during emergency closures or during inclement weather.
